To Register

To receive disability services, you must provide documentation verifying the disability and your need for accommodations. Verification information typically includes, but is not limited to:

  • A clear diagnosis from a qualified professional
  • Description of evaluation methods
  • Current limitations in the academic setting
  • Description of current/past accommodations
  • Medications and possible side effects
  • Possible accommodations needed

Steps to Take

Step 1: Complete and submit the Student Disability Services Request Form.

  • The form is available on Knowledgebase, titled “Student Disability Accommodation Registration.”
  • Include as much information as possible.
  • Upload documentation if available.

Step 2: Gather and submit documentation.

  • If documentation was not uploaded with the initial form submission, gather supporting documents from a physician, clinician, therapist, mental health provider or other qualified healthcare professional.
  • Documentation should generally include diagnosis/condition, current impact on academic functioning and recommended accommodations/support needs.
  • A university documentation form is available for providers if needed.
  • Submit documents to success@sf.edu or in person to the Cupertino Student Success Center.

Step 3: Schedule an intake appointment.

  • After documentation has been submitted, schedule a Student Disability Intake Meeting to discuss accommodation requests, academic barriers and support options.
  • Appointments may be virtual or in person.
  • Email success@sf.edu to schedule your Student Disability Intake Meeting.
