Reporting an IT Security Incident
If an IT incident is suspected, it is important to act as soon as possible. While you may think an incident is "no big deal" or just a "simple mistake", it could have serious implications for other university departments or the university as a whole. The University also has specific legal responsibilities in response to a confirmed or suspected security incident. If the suspected incident involves a possibly compromised system, then the system should be disconnected from the University network. This will minimize the threat to other university systems. If the incident requires immediate attention, contact Technology Security and Compliance at 260-399-7700 x6019. Otherwise, please fill out and submit the form below.
When investigating reported IT security incidents, the primary university goal is to isolate and fix the IT security problem or flawed process, rather than to assign blame to a specific individual. University computer and system users should not hesitate to report incidents, even if it is only suspected or if you believe it has already been resolved. Recent news regarding security incidents shows that it is far worse to ignore or attempt to cover up a security incident (only to have it discovered at a later date) than to deal with it immediately.
While including your contact information is helpful for follow-up information gathering, it is not required. This provides a way to anonymously report activities that may involve criminal, unethical, or otherwise inappropriate usage of the university's information technology resources.
Please fill out the form below to report an IT security incident. All fields are not required, and may be left blank if they do not apply to your situation.
