Appeal Form
Special Circumstances Forms
Students and families who experience a change in circumstances such as an involuntary loss of employment or unusual medical/dental expenses not covered by insurance may request a review of their financial aid file. Professional judgments are reserved for extreme situations and families must provide the appropriate documentation. Students must complete the Special Circumstance form below, provide required supporting documentation listed on the form, and complete the verification process by submitting verification worksheets (form available above), COPY of Federal Tax Return and COPY of W2's as indicated on the form.
Summer Aid Eligibility Form
This form must be completed to have the Office of Financial Aid review your eligibility for Summer Session(s). This form is available in March.
