CLINICAL REQUIREMENTS NEEDED FOR THE NURSING
PROGRAMS
ALL nursing students (LPN-ASN, ASN, BSN, MSN) must
complete all clinical requirements to be eligible to
attend clinical.
All students are required to have a health & physical
examination prior to admission to their first clinical
experience. The physical examination must be dated
within 1 year prior to admission to the first clinical
experience. Students must also submit an up-to-date
verification of their immunization profile as part of
the health and physical examination.
Student are required to receive a TB test annually
and have it read by a professional. Students who have a
positive reaction to the TB test are required to submit
documentation of one clear chest x-ray and a Chest X-ray
Reactor Form completed annually thereafter.
All nursing students are required to complete the
series of Hepatitis B vaccinations, or present proof of
adequate titer level of Hepatitis B vaccine, or sign a
waiver.
Each student is required to complete a CPR
certification course (adult, infant, child). Current CPR
certification must be maintained throughout the entire
program of study.
A Limited Criminal History Check must be completed
annually while enrolled in a nursing clinical course.
All students must sign the following forms annually:
USF Standard of Conduct and Confidentiality Agreement,
USF Communicable Disease Statement of Informed Consent,
USF Student Understanding, Parkview Hospital
Confidentiality Agreement, and Required Computer
Documentation Training Release form.
The student must complete all current requirements
mandated by each clinical agency yearly. Onsite clinical
orientation my be required at clinical agencies prior to
the start of the semester.
Students will receive uniform and equipment
requirements for clinicals. |
